Physiology of continence - Continence and micturition involve a balance between urethral closure and detrusor muscle activity. Urethral pressure normally exceeds bladder pressure, resulting in urine remaining in the bladder. The proximal urethra and bladder are both within the pelvis. Intraabdominal pressure increases (from coughing and sneezing) are transmitted to both urethra and bladder equally, leaving the pressure differential unchanged, resulting in continence. Normal voiding is the result of changes in both of these pressure factors:urethral pressure falls and bladder pressure rises.

I run long distance and am training for the DWD Marathon. The diaper I wear while running is a Pampers 6! Okay, at this point it doesn't make sense so I've got some explaining to do. I'm 5'9, 159 pounds and my waist is 32. Of course, I'm built mostly like a runner but not that skinny. I've made an elastic strap using 1.5 elastic and modified it to work with Pampers diapers. In the center, on the inside of the strap, are three squares of Aplix hook. One is in the center and the others are on either side, about 2away. These grap the outside front of the diaper like the back tabs are meant to. The other side of the strap, the outside, has a landing zone on each end to accept the Pampers tab. Half the tab is hook and the other half is adhesive. Aplix loop is positioned for the Pampers hook and the shiny side of a piece of PUL is sewn to accept the adhesive. The strap holds the diaper up securely and just barely high enough.

Adult diapers have to be supportive, hence why most adult diapers have either 4 or 6 tapes on the diaper because they are there to hold up the diaper and provide support from sagging. In addition, other factors that help prevent sagging in diapers is the core of the diaper. If the core of the diaper is made up good compacted material then it will also help support the diaper from sagging. One of the main reasons besides the features and absorbency that Bambino Diapers are my main diaper is because no matter how much I wet them, they never seem to sag on me that bad. Hence, no chafing! Sagging is one of the number one causes of chafing with adult diapers because the further it sags down, the more its going to rub on your legs and cause chafing.
